Forms are specific to each module: HSEQ, Workers, Equipment, and Contractors. To create and manage a form, first access the relevant module then choose one of 3 options: (1) create a form from scratch, import one from the NeXafe form library (which you can use as is or modify to your needs), or clone an existing form (that you can also modify). Once a form is created, it needs to be published before it can be integrated in Passports or Events. Forms can be edited at anytime (see Designing et editing forms below).

To create a form

From scratch
- Click the +Form 1️⃣ blue button then name your form and click Create.
- The new form will be open in edit mode.
- For HSEQ forms, you will have to select if you want to create a:
- Single Use form: once open a single form needs to be submitted and cannot be edited again.
- Multiple Use form: once open and submitted once, a multiple form remain openable by multiple users for edition until marked Closed and submitted.
By importing a form from the library
- Click the +Ready-to-use Form 2️⃣ blue button then the + blue button of the selected form and rename it.
- Before importing a form, you can also:
- Read the description of a form by clicking the i icon.
- View a form by clicking the Eye icon.
- Before importing a form, you can also:
- Once the form is added to your list, click it to open it in edit mode.
By cloning an existing form
- Click the Duplicate 3️⃣ icon at the end of the form row.
- Once cloned, the new form will be in your list under the name “Clone – [original form name]”.
- Click it to open it in edit mode.
To publish a form
- Click open a form in edit mode then click on the Publish yellow-orange button.
- Choose if you want to notify users of the publishing.
- Once a form is published, you can:
- Fill it (ideal for testing) by clicking on the Fill out ▶ icon of the form in the Form table.
- Add it to a Passport or an Event.
To delete a form
- A form must be Unpublished to be deleted.
- Click open a published form in edit mode then click on the Unpublish green button.
- Click on the red Trash can icon at the end of the row of the unpublished form to delete and confirm.
- Make sure that the form is not a Passport item or integrated in an Event.
- Submitted forms will remain in their location with their content.
- If a delete form is still active in a Passport or an Event to be filled, it will not be accessible when clicked by the user.
